Book an appointment to register your child and reserve a space for him or her in our programs – Space is limited!. Registrations are on a first-come, first-serve basis. Registrations at Bright Eyes Kids are opened year round based on availability.
Registration Forms
The registration forms are available to download from the link below for the parents to complete at home. Registration forms are also available to pick-up from our child care center at 14814 Piuma Ave, Norwalk, CA 90650.
All registration information must be completed and signed by the parent(s), together with all updated immunization records prior to the start of a child’s attendance. If more than one child is enrolled per family, a separate registration form must be completed for each child.
